Here’s how to fix them.
A missing birthplace address or incorrect information on your PSA birth certificate could delay your passport application. Fortunately, there are ways to correct these errors without necessarily starting the entire application process again.
A viral Threads post recently highlighted the problem after a netizen complained that a sibling’s passport application could not proceed because the birth certificate identified “Ospital ng Maynila” but lacked complete birthplace details.
“Di ba pwedeng iconsider na lang to kasi iisa lang naman ang Ospital ng Maynila?” asked user @alexanduhhhhh_.
Other netizens shared similar experiences, including passport applications delayed over incomplete city names and birthplace information.
So what should applicants do?
First, check your original birth record
Visit the Local Civil Registrar (LCR) of the city or municipality where your birth was registered.
Request a certified true copy of your original Certificate of Live Birth.
If the original contains complete information missing from the PSA copy, ask the LCR how to reconcile the records.
The DFA may require additional supporting documents, so confirm whether the certified copy will be accepted before returning.
If information is missing, file a supplemental report
If the original record itself lacks the birthplace details, ask the LCR about filing a supplemental report.
This procedure supplies information omitted during the original registration.
Applicants may need hospital records, baptismal certificates, school records, or other documents establishing the correct birthplace.
Once processed, coordinate with the PSA regarding the updated record.
If information is wrong, request a correction
If the birthplace contains an incorrect entry rather than missing information, a petition for correction of clerical error under Republic Act No. 9048 may be appropriate.
The LCR will determine whether the error qualifies for administrative correction and explain the required documents and fees.
Applicants may need a PSA-issued annotated birth certificate before passport processing can continue.
What about your DFA appointment?
If the DFA flags a discrepancy, request written instructions identifying the documents needed.
Ask whether your application can remain pending while the issue is resolved, how long you have to comply, and whether another appointment or payment is necessary.
Do not assume that every applicant automatically receives a 30-day extension.
How long will correction take?
A certified true copy may be available relatively quickly, depending on the LCR.
Supplemental reports and clerical corrections can take longer, particularly when updated records must be transmitted to and reflected in the PSA system.
Fees and processing times vary by procedure and locality.
The lesson is simple: Before booking a passport appointment, examine your PSA birth certificate carefully.
A missing city, incomplete birthplace, or incorrect entry could mean additional paperwork, unexpected expenses, and weeks or months of delays.
