I’m both an HIV primary care doctor and a regenerative medicine doctor. For me, caring for PLHIV is not just about viral suppression. In my clinical work at Hara Clinic where I serve as Medical Director, I talk to my patients about what happens even when they are undetectable.

Understanding the “undetectable” milestone

“Undetectable” means the amount of HIV in the blood is so low that it cannot be measured by standard tests. This is typically achieved through consistent use of antiretroviral therapy, which suppresses viral replication and prevents disease progression and transmission. While being undetectable is a major success of modern HIV care, it does not erase the long-term effects of chronic infection, which is why attention to overall health and quality of life still matters.

Addressing the gap between viral suppression and vitality

Many PLHIV experience persistent fatigue, brain fog, low libido, or poor recovery. Their labs look fine, and viral load is controlled. Yet the body still feels off. That doesn’t mean treatment is failing. It means the immune system has been working overtime for years.

The role of NAD⁺ and cellular health

Chronic immune activation causes oxidative stress, which depletes NAD⁺, a molecule your cells need for energy production and repair. Low NAD⁺ doesn’t cause HIV, but it can make daily life feel heavier than it needs to be.

This matters to me because being undetectable should not mean settling for “just okay.” My PLHIV patients deserve energy, clarity, intimacy, and a good quality of life.

A regenerative approach to HIV wellness

So when someone is undetectable but consistently tired or not feeling their best, we talk about supportive options from a wellness and regenerative standpoint, always as an adjunct to proper HIV care.
